Getting Started with Your QualChoice Plan

Your Member ID Card

Carry your member ID card with you any time you seek medical care from a:

  • Doctor
  • Pharmacy
  • Hospital
  • Other healthcare provider

The effective date is the day you can start using your insurance to see your doctor and get prescriptions filled. For emergencies while travelling or outside our service area, call the phone number listed on your card for the QualChoice National Network.

Your plan does not cover any costs until: 1) the effective date of your policy; and 2) QualChoice has received and applied your first premium payment.

Finding a Doctor

Doctors and hospitals in our network have a special arrangement with QualChoice. Your out-of-pocket costs will be lower if you use an in-network medical provider.

Paying Your Individual Plan Premium

If you’re on an individual plan, don't let your coverage lapse! Your payment must be received (not just postmarked) by or on the due date listed on your invoice.
